Create online forms that allow users to look up and select values.

  1. Step 1: Choose a ready-made template (such as a check-in or leave request form) or start with a blank slate, then enter the Form Builder to add form fields and edit the design. Choose from 20+ form field types and a useful selection of form design options including images, logos, formatted text, colours and fonts.
  2. Step 2: Add a Lookup field for each data point and configure the options. Add matching fields from your database form that you want to be automatically populated.
  3. Step 3: For Autosuggest Lookup forms, such as address autocomplete fields, check the 'Suggest possible values as the user types' option, and save. Use advanced features such as conditional logic to show or hide the lookup field based on the user's answers.
