Case Study: Motivate Kids Occupational Therapy


Finding an online forms solution that integrated seamlessly with coreplus was a remarkably simple exercise for children’s occupational therapy practice, Motivate Kids.

Integrations with coreplus are a major factor in the digital health practice management software’s appeal.

For Rochelle Mutton, Director and founder of children’s occupational therapy practice, Motivate Kids, it was Snapforms’ seamless integration with coreplus that initially piqued her interest when she was looking for an online forms solution.

The Snapforms integration means it goes straight into coreplus.

“We were using a different sort of electronic form,” recalls Rochelle.

“But the Snapforms integration means it goes straight into coreplus – making it more automatic, which was what we wanted.”

What was the business challenge?

Rochelle and her team of around 25 occupational therapists and support staff see around 300 children each week at their Adelaide-based practice.

Their clients’ privacy is an integral part of Motivate Kids’ service as an NDIS-registered provider, so the fact that Snapforms securely stores all data here in Australia was a major contributing factor in Rochelle’s choice of online forms provider.

Our families could start the questionnaire and come back to it, whereas with our previous one they had to do it in one sitting.

But it’s also the complexity of many of the forms they use in occupational therapy (OT) that influenced Rochelle’s decision to go with Snapforms.

“We liked Snapforms because our families could start the questionnaire and come back to it, whereas with our previous one they had to do it in one sitting,” explains Rochelle.

Snapforms’ save and resume functionality allows users to save their progress and come back to complete their form later.

Allowing people to continue filling in the form later has been shown to increase submission rates and increase the quality of responses received.

“The forms can be pretty big. And that was a frustration for families. So that was a benefit for us and also for families.”

How was the solution implemented?

When it came to implementation, Rochelle’s team reached out to Snapforms for assistance creating the forms they wanted.

We currently use six forms and Snapforms helped with all six of them.

We got Snapforms to do an example one, just so that I could see what it was, and then I sent them my Word Documents and got the Snapforms team to create them,” says Rochelle.

“They did an amazing job, it was really well worth saving the time.

“We currently use six forms and Snapforms helped with all six of them.

“It’s saving time, it’s more automated, and we haven’t had any complaints about families starting the forms and then not being able to finish them and needing to repeat them, so that’s really positive.”

Rochelle’s team also uses Snapforms to collect parents’ and carers’ signatures for consent to certain conditions.

“We use it for our client profile form,” explains Rochelle.

“Which means they acknowledge their fee is payable on the day, or they agree to have their child on social media and things like that.”

I think they look great and they’re easy to edit.

As far as ease-of-use and styling of their forms goes, Rochelle says she and her team are very happy with Snapforms.

“I think they look great and they’re easy to edit when we want to add a few extra things or make it so some things are required to be able to submit the form – so that’s been really easy to do.”

As for the future, Rochelle says she and her team are looking forward to exploring all the opportunities the coreplus Snapforms integration makes possible.

“We’re always looking for ways of using technology in more innovative ways and capturing content differently,

“So in the future with all the forms that we get filled in – we’d look to see if there’s a way that we can pull a report that shows how many children were Caesarian, or how many children are this or that, to pull some statistics or insightful trends,” says Rochelle.
